¤ Assist older adults and their families in determining the ideal living situation for their needs

¤ Maintain communication between the older adults or dependents and their out-of-state relatives

¤ Evaluate the elder's physical, environmental, nutritional, psychological, safety, and social needs

¤ Assist in locating in-home services to meet personal and/or special medical needs

¤ Monitor changing needs and act as liaison between client, physician, and family

¤ Provide home, hospital, and facility visits

¤ Guide in the selection of alternative living arrangements including: Adult Day Health Care, Residential Care Facilities, Immediate Care Facilities, or Skilled Nursing Facilities

¤ Coordinate with physicians, attorneys, financial planners and state agencies to develop the plan of care

¤ Present at employer or employee group, club, and organization lectures
