Will act as health care guardian/conservator for elders without immediate
family.
Assisting older adults and their families in determining the ideal living
situation catered to their needs while maintaining a stimulating and active
lifestyle.
A liaison between
the older adults or dependents and their out of state relatives.
Comprehensive evaluation of individual physical, environmental, nutritional,
psychological, safety, and social needs.
Assistance in locating in-home services to meet personal and or special
medical needs.
Monitor changing needs and act as liaison between client, physician, and
family.
Home, Hospital, and Nursing Home Visits.
Guidance in the selection of alternative living arrangements including:
Adult Day Health Care, Residential Care Facilities, Immediate Care Facilities,
or Skilled Nursing Facilities.
Coordination with
Physicians, Attorneys, Financial Planners, and State Agencies to develop
the plan of care.
Employer or Employee Group, Club, and Organization lectures.
