Fees - Care Management Services

Care Management Services are not covered under Medicare, but may be covered under a long-term care insurance plan.

The initial comprehensive on-site assessment is billed as a one time, all inclusive fee to include the assessment, individualized care plan, referral and arrangement of services.

Care management fees are billed per hour in 5 minute increments. We will keep you informed of the services provided, time spent and important information through communication provided via written reports, email or telephone calls.

We will discuss and review the care plan with you and determine the amount of services/time to be provided that is cost effective and meets your budget needs.

Care management services are designed to create the most cost-effective plan. Our services can save you money in the long run by assessing your precise needs and arranging for services to be delivered in the home rather than requiring a more costly move to a retirement or nursing facility.
